One poorly conducted study found, interestingly enough, that the epidural is NOT correlated with clavicular fractures – although they excluded any baby who was not in the occiput anterior position. The authors had 403 cases of clavicular fracture, and ruled out 316 cases because of positioning. Which brings me directly to the study done by Dr. Ellice Lieberman et al that found the epidural significantly associated with malpositioned babies, specifically occiput posterior, or sunny-side-up. This finding was particularly of interest, because these baby’s were not found to necessarily be posterior at the beginning or midpoints of the labor. Nor were the mom’s reporting back labor, or a labor with more pain scale intensity than the mom’s who did not get an epidural. In other words, they weren't more likely to get an epidural because of their malpositioned baby.I think looking at a combination of the two would be an amazing study!
